The HMO Silver Network is available in all or parts of Kern, Los Angeles, Orange, Riverside, San Bernardino, San Diego, San Francisco and Ventura counties.

Exclusive EOA products offer your employees reliable, convenient HMO coverage, plus the flexibility of self-referral access to specialists (physicians, practitioners and health professionals) for specified services in statewide PPO network.

Your employees choose between two tiers of benefits – HMO or PPO. For this flexibility, they pay a little bit more when using their tier 2 PPO self-referral benefit.

EOA Products Provider Network Options

EOA products are available with:

  • Full HMO network
  • Silver Network – a select subset of full HMO network, available in Kern, Los Angeles, Orange, Riverside, San Bernardino, San Diego, San Francisco and Ventura counties. Health Net's PPO is a network of more than 44,000 physicians statewide. You may select any physician at any time from the network, or you can see physicians outside the PPO network for a higher cost.

What are the Basics of a Point of Service plan?

With the Point of Service plan, you can choose between Preferred Provider Organization (PPO) and Out-of-Network benefit levels every time you need care.

  • With the PPO benefit level, you can see any physician or specialist within the Health Net provider network without a referral any time you need care.
  • The Out-of-Network benefit level lets you seek care from any out-of-network physician or specialist without a referral.

Consult your Evidence of Coverage or Certificate of Insurance ("Plan Documents") for specific coverage and limitations. The fact that a medication is listed on a drug list does not guarantee that your physician will prescribe it for you for a particular medical condition.

Since our drug lists below are updated regularly, they are subject to change. To access your specific pharmacy plan benefits, view the most current drug lists, order drugs online, and more, you must register and/or log in and then select Searchable Drug List under View Prescription Coverage/Your Drug List.
